Má tá tú ag déanamh iarratas ar chlárú sa rang speisialta ASD, ní mór duit an fhoirm iarratais seo a líonadh isteach agus ní mór don tuismitheoir/caomhnóir gach tuairisc atá ar fáil a chur i scríbhinn chuig an bPriomhoide, laistigh den am atá san fhógra iontrála a bhaineann leis an scoil. Caithfidh tuairisc ildisciplíneach a bheith ag an pháiste le diagnóis (DSM IV nó V) agus moladh ann ó thaobh freastal ar rang ASD atá ceangailte le scoil phríomhshrutha. Is féidir é a chur chuig: An Príomhoide, Gaelscoil Uí Earcáin, Bóthar Ghlas an Éin, Fionnghlas, Baile Átha Cliath 11, D11VP44.
If you wish to apply for enrolment in the ASD special class, you need to fill in this application form for enrolment and all available reports need to be submitted to the school by the parent/guardian, within the time-frame specified in the admissions notice pertaining to the upcoming school year. The child must have a multi-disciplinary report with a diagnosis of a qualifying Autism Spectrum Disorder (DSM IV or V) and a recent recommendation to attend an ASD class attached to mainstream school. These documents must be sent to: An Príomhoide, Gaelscoil Uí Earcáin, Bóthar Ghlas an Éin, Fionnghlas, Baile Átha Cliath 11, D11VP44.
